(This page was unashamedly aimed at a North American audience; however, I'm getting more liberal, which means that since I've found some non-North American hatcheries, I've listed them.)
I'm not recommending any of these in particular. Over the years I've collected catalogs and have bought from many of these houses, but not all. None of the ones I've used has done me wrong, so to speak. I'm putting this list here to aid folks who are looking for birds and supplies. They are now in alphabetical order by state for the US, followed by other countries. Those with a green bullet I have used at least once. Those with black bullets I've no experience with. I'll annotate as the spirit moves me.
It's a good idea to make sure that any chicks are shipped on Monday or Tuesday so they can't get stuck over the weekend.

Hatcheries (many of these also sell equipment)
They seem to have broilers, some kinds of layers, and turkeys.
Specializes in non-flighty production Leghorns, also various other Leghorns and some Old English Games.
They raise pheasants (Ringnecks, Blue-eareds, Goldens, Silvers, Swinhoes, Lady Amhersts, Reeves and Impeyans), Chukars, Bobwhite and Pharoah Quail, and Blue India Peafowl.
A family-run operation that maintains its own breeding flocks. Belt handles a bunch of standard breeds, a few bantams, and a couple breeds each of duck and turkey.

Patrick Kelley's hatchery has Welsummer, RIR, Cubalayas, Shamo, Chantecler, Marans, Asil, Iowa blue, Java, Yokohama, Saipan, Orloff and White Giant chickens.
They have pictures on the web of most of the breeds they handle and their catalog has nice color photos of most breeds, also.

Their birds are not necessarily show stock, but usually pretty good. They also sell poultry equipment, but I find their prices to be relatively high on this stuff. Stromberg's supplies many varieties of domestic poultry and also deals in pheasants and wild waterfowl. They handle adult stock as well as day-olds. I understand that they do not keep stock, but act as a middleman, finding whatever you want. They also sell supplies and books. I find their prices quite high. I think that the books published under their own name are for the most part ghastly. They are sloppily put together, unindexed, and mostly anecdotal with minimal references (see my review of Poultry of the World). On the other hand, when they screwed up on an order for me and sent some adult birds that cost $100 more than I expected (having ordered juveniles), they were very good about it. They told me to keep the birds and they swallowed the difference. I do note that this was back in 1988 -- can't say how they are now.

Claude McAllister, who owns this place, raises many varieties of quail, as well as bantam ducks and some bantam chickens, plus true Araucanas in large and bantam. He also carries lots of poultry equipment.

I've included this one because they sell crates for shipping adult birds via Express Mail. Note that the standard Horizon boxes are filtered for shipping pheasants and waterfowl, etc., but that you can use the Petporters for shipping chickens. They also sell an egg-shipping box.

I just learned that the ABA has a chicken calendar which has pictures of champion bantams. It costs $6 including shipping and can be ordered from Don Karasek, 2504 State Rd 133, Blue River, WI 53518.
Yet another chicken calendar, The Cluckendar has some of the poorest (not worst, just poorest) poultry puns I've ever encountered. But it does have pictures of chickens, some quite cute.
