(This page was unashamedly aimed at a North American audience; however, I'm getting more liberal, which means that since I've found some non-North American hatcheries, I've listed them.)
I'm not recommending any of these in particular. Over the years I've collected catalogs and have bought from many of these houses, but not all. None of the ones I've used has done me wrong, so to speak. I'm putting this list here to aid folks who are looking for birds and supplies. They are now in alphabetical order by state for the US, followed by other countries. Those with a green bullet I have used at least once. Those with black bullets I've no experience with. I'll annotate as the spirit moves me.
It's a good idea to make sure that any chicks are shipped on Monday or Tuesday so they can't get stuck over the weekend.

Hatcheries (many of these also sell equipment)
They raise pheasants (Ringnecks, Blue-eareds, Goldens, Silvers, Swinhoes, Lady Amhersts, Reeves and Impeyans), Chukars, Bobwhite and Pharoah Quail, and Blue India Peafowl.
A family-run operation that maintains its own breeding flocks. Belt handles a bunch of standard breeds, a few bantams, and a couple breeds each of duck and turkey.
I understand that they're no longer selling layers, just meat birds, and possibly still Pekin and Rouen ducks, and commercial turkeys.
They have pictures on the web of most of the breeds they handle and their catalog has nice color photos of most breeds, also.
They have some large fowl, geese and peafowl. Seem to specialize in ducks, especially Calls.
They are willing to ship as few as 3 chicks and handle a fairly large assortment of breeds. (I have been told that they'll even sex bantam chicks.) They do not have a storefront and all orders must be shipped. They also sell equipment and various poultry-related gift items.
They specialize in four types of chickens: Buff Orpingtons, Rhode Island Reds, White Leghorns and Black Sex-links.
Doms, RIRs, Orps, NH Reds, Ameraucanas, Favs, Buckeyes, Delawares, Welsummers, Silkies, Red Broilers, Cobb 500s, and ISA Browns.
Quite a few production breeds, broilers, Wyandottes, some bantams, Broad-breasted turkeys (Bronze and White) some ducks, some geese, ringneck Pheasants, Chukars, Mallards, and Rio Grande Wild Turkeys plus some equipment. they also sell hatching eggs of many of their breeds.

They've got exhibition and bantam fowl.
They only handle rare and heritage varieties of turkeys, including some varieties that they've been developing.

I've ordered from McMurray many times. They have many breeds of chickens( both standard and bantam), plus ducks, geese, guineas, pheasants, turkeys, peafowl and quail. Their birds are not necessarily show stock, but usually pretty good. They also sell poultry equipment, but I find their prices to be relatively high on this stuff. BTW, their catalog is the best free guide to what various breeds look like that you'll easliy find. (Of course, I hope that when I get enough images, these pages will be an even better source.)

Stromberg's supplies many varieties of domestic poultry and also deals in pheasants and wild waterfowl. They handle adult stock as well as day-olds. I understand that they do not keep stock, but act as a middleman, finding whatever you want. They also sell supplies and books. I find their prices quite high. I think that the books published under their own name are for the most part ghastly. They are sloppily put together, unindexed, and mostly anecdotal with minimal references (see my review of Poultry of the World). On the other hand, when they screwed up on an order for me and sent some adult birds that cost $100 more than I expected (having ordered juveniles), they were very good about it. They told me to keep the birds and they swallowed the difference. I do note that this was back in 1988 -- can't say how they are now.

This hatchery specializes in domestic waterfowl. They will ship exhibition quality stock if requested. [ed. note: I love their Runner Ducks. But I've had bad experiences with their geese and what bothers me most is that they will not respond to my emails. I wrote them in July '11 and again in Aug. '11. Have not had the courtesy of a response to either communication as of mid-October.]

The VSB Electronic Doorkeeper is a battery-powered unit which can be set to open and close your coop doors to let the birds out in the AM and close them in at dusk.

Poultry-related Gifts and other odd stuff
They carry many poultry-related decals and will make others to order.
A nice chicken calendar, plus many other rural and not-so-rural calendars. I do
have one problem with their calendar, in that I like to use pencil, not pen, and it doesn't take that
very well. Great pictures, tho'. I just learned that the ABA has a chicken calendar
which has pictures of champion bantams. It costs $6 including shipping and can be ordered from Don
Karasek, 2504 State Rd 133, Blue River, WI 53518.
Yet another chicken calendar, The Cluckendar has some of the poorest (not
worst,
just poorest) poultry puns I've ever encountered. But it does have pictures of chickens, some quite
cute.
Medications
Poultry oriented, and the guy who runs it knows his stuff!
They carry some poultry supplies and medications along with
the same for most livestock and pets. This is a large catalog.
Their catalog seems to be a one page listing of many kinds of
poultry medications.
